By electronically signing below and as parent or legal guardian of the student(s) listed below, I hereby authorize New Life Church and it's designated individuals to transport my student(s), to and from the activity selected below.  

By electronically signing below, I also agree to hold harmless, New Life Church and it's designated individuals from any and all claims, losses or injury resulting from the transportation of my student to and from the event and activities while participating in selected event below.
